The usbfs API has a peculiar hole: Users are not allowed to reap their
URBs after the device has been disconnected. There doesn't seem to be
any good reason for this; it is an ad-hoc inconsistency.
The patch allows users to issue the USBDEVFS_REAPURB and
USBDEVFS_REAPURBNDELAY ioctls (together with their 32-bit counterparts
on 64-bit systems) even after the device is gone. If no URBs are
pending for a disconnected device then the ioctls will return -ENODEV
rather than -EAGAIN, because obviously no new URBs will ever be able
to complete.
The patch also adds a new capability flag for
USBDEVFS_GET_CAPABILITIES to indicate that the reap-after-disconnect
feature is supported.

If updating journal superblock fails after journal data has been
flushed, the error is omitted and this will mislead the caller as a
normal case. In ocfs2, the checkpoint will be treated successfully
and the other node can get the lock to update. Since the sb_start is
still pointing to the old log block, it will rewrite the journal data
during journal recovery by the other node. Thus the new updates will
be overwritten and ocfs2 corrupts. So in above case we have to return
the error, and ocfs2_commit_cache will take care of the error and
prevent the other node to do update first. And only after recovering
journal it can do the new updates.

Introduce KEXEC_CONTROL_MEMORY_GFP to allow the architecture code
to override the gfp flags of the allocation for the kexec control
page. The loop in kimage_alloc_normal_control_pages allocates pages
with GFP_KERNEL until a page is found that happens to have an
address smaller than the KEXEC_CONTROL_MEMORY_LIMIT. On systems
with a large memory size but a small KEXEC_CONTROL_MEMORY_LIMIT
the loop will keep allocating memory until the oom killer steps in.

->auto_asconf_splist is per namespace and mangled by functions like
sctp_setsockopt_auto_asconf() which doesn't guarantee any serialization.
Also, the call to inet_sk_copy_descendant() was backuping
->auto_asconf_list through the copy but was not honoring
->do_auto_asconf, which could lead to list corruption if it was
different between both sockets.
This commit thus fixes the list handling by using ->addr_wq_lock
spinlock to protect the list. A special handling is done upon socket
creation and destruction for that. Error handlig on sctp_init_sock()
will never return an error after having initialized asconf, so
sctp_destroy_sock() can be called without addrq_wq_lock. The lock now
will be take on sctp_close_sock(), before locking the socket, so we
don't do it in inverse order compared to sctp_addr_wq_timeout_handler().
Instead of taking the lock on sctp_sock_migrate() for copying and
restoring the list values, it's preferred to avoid rewritting it by
implementing sctp_copy_descendant().
Issue was found with a test application that kept flipping sysctl
default_auto_asconf on and off, but one could trigger it by issuing
simultaneous setsockopt() calls on multiple sockets or by
creating/destroying sockets fast enough. This is only triggerable
locally.

dst_orig should be released on error. Function like __xfrm_route_forward()
expects that behavior.
Since a recent commit, xfrm_lookup() may also be called by xfrm_lookup_route(),
which expects the opposite.
Let's introduce a new flag (XFRM_LOOKUP_KEEP_DST_REF) to tell what should be
done in case of error.

When the LPM policy is set to ATA_LPM_MAX_POWER, the device might
generate a spurious PHY event that cuases errors on the link.
Ignore this event if it occured within 10s after the policy change.
The timeout was chosen observing that on a Dell XPS13 9333 these
spurious events can occur up to roughly 6s after the policy change.

A non-percpu VIRQ (e.g., VIRQ_CONSOLE) may be freed on a different
VCPU than it is bound to. This can result in a race between
handle_percpu_irq() and removing the action in __free_irq() because
handle_percpu_irq() does not take desc->lock. The interrupt handler
sees a NULL action and oopses.
Only use the percpu chip/handler for per-CPU VIRQs (like VIRQ_TIMER).

The range check for b-tree level parameter in nilfs_btree_root_broken()
is wrong; it accepts the case of "level == NILFS_BTREE_LEVEL_MAX" even
though the level is limited to values in the range of 0 to
(NILFS_BTREE_LEVEL_MAX - 1).
Since the level parameter is read from storage device and used to index
nilfs_btree_path array whose element count is NILFS_BTREE_LEVEL_MAX, it
can cause memory overrun during btree operations if the boundary value
is set to the level parameter on device.
This fixes the broken sanity check and adds a comment to clarify that
the upper bound NILFS_BTREE_LEVEL_MAX is exclusive.

Constantly forking task causes unlimited grow of anon_vma chain. Each
next child allocates new level of anon_vmas and links vma to all
previous levels because pages might be inherited from any level.
This patch adds heuristic which decides to reuse existing anon_vma
instead of forking new one. It adds counter anon_vma->degree which
counts linked vmas and directly descending anon_vmas and reuses anon_vma
if counter is lower than two. As a result each anon_vma has either vma
or at least two descending anon_vmas. In such trees half of nodes are
leafs with alive vmas, thus count of anon_vmas is no more than two times
bigger than count of vmas.
This heuristic reuses anon_vmas as few as possible because each reuse
adds false aliasing among vmas and rmap walker ought to scan more ptes
when it searches where page is might be mapped.

Looks like audigy emu10k2 (probably emu10k1 - sb live too) support two
modes for DMA. Second mode is useful for 64 bit os with more then 2 GB
of ram (fixes problems with big soundfont loading)
1) 32MB from 2 GB address space using 8192 pages (used now as default)
2) 16MB from 4 GB address space using 4096 pages
Mode is set using HCFG_EXPANDED_MEM flag in HCFG register.
Also format of emu10k2 page table is then different.

Sasha Levin found a NULL pointer dereference that is due to a missing
page table lock, which in turn is due to the pmd entry in question being
a transparent huge-table entry.
The code - introduced in commit 1998cc048901 ("mm: make
madvise(MADV_WILLNEED) support swap file prefetch") - correctly checks
for this situation using pmd_none_or_trans_huge_or_clear_bad(), but it
turns out that that function doesn't work correctly.
pmd_none_or_trans_huge_or_clear_bad() expected that pmd_bad() would
trigger if the transparent hugepage bit was set, but it doesn't do that
if pmd_numa() is also set. Note that the NUMA bit only gets set on real
NUMA machines, so people trying to reproduce this on most normal
development systems would never actually trigger this.
Fix it by removing the very subtle (and subtly incorrect) expectation,
and instead just checking pmd_trans_huge() explicitly.

It is reported that on a physically 64-bit addressed machine, 32-bit kernel
can trigger crashes in accessing the memory regions that are beyond the
32-bit boundary. The region field's start address should still be 32-bit
compliant, but after a calculation (adding some offsets), it may exceed the
32-bit boundary. This case is rare and buggy, but there are real BIOSes
leaked with such issues (see References below).
This patch fixes this gap by always defining IO addresses as 64-bit, and
allows OSPMs to optimize it for a real 32-bit machine to reduce the size of
the internal objects.
Internal acpi_physical_address usages in the structures that can be fixed
by this change include:
1. struct acpi_object_region:
acpi_physical_address address;
2. struct acpi_address_range:
acpi_physical_address start_address;
acpi_physical_address end_address;
3. struct acpi_mem_space_context;
acpi_physical_address address;
4. struct acpi_table_desc
acpi_physical_address address;
See known issues 1 for other usages.
Note that acpi_io_address which is used for ACPI_PROCESSOR may also suffer
from same problem, so this patch changes it accordingly.
For iasl, it will enforce acpi_physical_address as 32-bit to generate
32-bit OSPM compatible tables on 32-bit platforms, we need to define
ACPI_32BIT_PHYSICAL_ADDRESS for it in acenv.h.
Known issues:
1. Cleanup of mapped virtual address
In struct acpi_mem_space_context, acpi_physical_address is used as a virtual
address:
acpi_physical_address mapped_physical_address;
It is better to introduce acpi_virtual_address or use acpi_size instead.
This patch doesn't make such a change. Because this should be done along
with a change to acpi_os_map_memory()/acpi_os_unmap_memory().
There should be no functional problem to leave this unchanged except
that only this structure is enlarged unexpectedly.

This patch fixes a bug for COMPARE_AND_WRITE handling with
fabrics using SCF_PASSTHROUGH_SG_TO_MEM_NOALLOC.
It adds the missing allocation for cmd->t_bidi_data_sg within
transport_generic_new_cmd() that is used by COMPARE_AND_WRITE
for the initial READ payload, even if the fabric is already
providing a pre-allocated buffer for cmd->t_data_sg.
Also, fix zero-length COMPARE_AND_WRITE handling within the
compare_and_write_callback() and target_complete_ok_work()
to queue the response, skipping the initial READ.
This fixes COMPARE_AND_WRITE emulation with loopback, vhost,
and xen-backend fabric drivers using SG_TO_MEM_NOALLOC.

Every USB Host controller should use this new
macro to define for how long resume signalling
should be driven on the bus.
Currently, almost every single USB controller
is using a 20ms timeout for resume signalling.
That's problematic for two reasons:
a) sometimes that 20ms timer expires a little
before 20ms, which makes us fail certification
b) some (many) devices actually need more than
20ms resume signalling.
Sure, in case of (b) we can state that the device
is against the USB spec, but the fact is that
we have no control over which device the certification
lab will use. We also have no control over which host
they will use. Most likely they'll be using a Windows
PC which, again, we have no control over how that
USB stack is written and how long resume signalling
they are using.
At the end of the day, we must make sure Linux passes
electrical compliance when working as Host or as Device
and currently we don't pass compliance as host because
we're driving resume signallig for exactly 20ms and
that confuses certification test setup resulting in
Certification failure.

When I added pfmemalloc support in build_skb(), I forgot netlink
was using build_skb() with a vmalloc() area.
In this patch I introduce __build_skb() for netlink use,
and build_skb() is a wrapper handling both skb->head_frag and
skb->pfmemalloc
This means netlink no longer has to hack skb->head_frag

It is curently possible to run a VM with architected timers support
without creating an in-kernel VGIC, which will result in interrupts from
the virtual timer going nowhere.
To address this issue, move the architected timers initialization to the
time when we run a VCPU for the first time, and then only initialize
(and enable) the architected timers if we have a properly created and
initialized in-kernel VGIC.
When injecting interrupts from the virtual timer to the vgic, the
current setup should ensure that this never calls an on-demand init of
the VGIC, which is the only call path that could return an error from
kvm_vgic_inject_irq(), so capture the return value and raise a warning
if there's an error there.
We also change the kvm_timer_init() function from returning an int to be
a void function, since the function always succeeds.

Ensure that other operations which raced with our setattr RPC call
cannot revert the file attribute changes that were made on the server.
To do so, we artificially bump the attribute generation counter on
the inode so that all calls to nfs_fattr_init() that precede ours
will be dropped.
The motivation for the patch came from Chuck Lever's reports of readaheads
racing with truncate operations and causing the file size to be reverted.

cancel[_delayed]_work_sync() are implemented using
__cancel_work_timer() which grabs the PENDING bit using
try_to_grab_pending() and then flushes the work item with PENDING set
to prevent the on-going execution of the work item from requeueing
itself.
try_to_grab_pending() can always grab PENDING bit without blocking
except when someone else is doing the above flushing during
cancelation. In that case, try_to_grab_pending() returns -ENOENT. In
this case, __cancel_work_timer() currently invokes flush_work(). The
assumption is that the completion of the work item is what the other
canceling task would be waiting for too and thus waiting for the same
condition and retrying should allow forward progress without excessive
busy looping
Unfortunately, this doesn't work if preemption is disabled or the
latter task has real time priority. Let's say task A just got woken
up from flush_work() by the completion of the target work item. If,
before task A starts executing, task B gets scheduled and invokes
__cancel_work_timer() on the same work item, its try_to_grab_pending()
will return -ENOENT as the work item is still being canceled by task A
and flush_work() will also immediately return false as the work item
is no longer executing. This puts task B in a busy loop possibly
preventing task A from executing and clearing the canceling state on
the work item leading to a hang.

This patch removes the possible hang by updating __cancel_work_timer()
to explicitly wait for clearing of CANCELING rather than invoking
flush_work() after try_to_grab_pending() fails with -ENOENT.

Right now, the migration code in migrate_page_copy() uses copy_huge_page()
for hugetlbfs and thp pages:

So, yay for code reuse. But:

and a non-hugetlbfs page has no page_hstate(). This works 99% of the
time because page_hstate() determines the hstate from the page order
alone. Since the page order of a THP page matches the default hugetlbfs
page order, it works.
But, if you change the default huge page size on the boot command-line
(say default_hugepagesz=1G), then we might not even *have* a 2MB hstate
so page_hstate() returns null and copy_huge_page() oopses pretty fast
since copy_huge_page() dereferences the hstate:

Mel noticed that the migration code is really the only user of these
functions. This moves all the copy code over to migrate.c and makes
copy_huge_page() work for THP by checking for it explicitly.

We have a race condition between move_pages() and freeing hugepages, where
move_pages() calls follow_page(FOLL_GET) for hugepages internally and
tries to get its refcount without preventing concurrent freeing. This
race crashes the kernel, so this patch fixes it by moving FOLL_GET code
for hugepages into follow_huge_pmd() with taking the page table lock.
This patch intentionally removes page==NULL check after pte_page.
This is justified because pte_page() never returns NULL for any
architectures or configurations.
This patch changes the behavior of follow_huge_pmd() for tail pages and
then tail pages can be pinned/returned. So the caller must be changed to
properly handle the returned tail pages.
We could have a choice to add the similar locking to
follow_huge_(addr|pud) for consistency, but it's not necessary because
currently these functions don't support FOLL_GET flag, so let's leave it
for future development.

The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.
That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works. However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.
In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV. And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.
However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d4514 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space. And user space really
expected SIGSEGV, not SIGBUS.
To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it. They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.
This is the mindless minimal patch to do this. A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.
Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

This patch addresses the original PR_APTPL_BUF_LEN = 8k limitiation
for write-out of PR APTPL metadata that Martin has recently been
running into.
It changes core_scsi3_update_and_write_aptpl() to use vzalloc'ed
memory instead of kzalloc, and increases the default hardcoded
length to 256k.
It also adds logic in core_scsi3_update_and_write_aptpl() to double
the original length upon core_scsi3_update_aptpl_buf() failure, and
retries until the vzalloc'ed buffer is large enough to accommodate
the outgoing APTPL metadata.

the following error pops up during "testusb -a -t 10"
| musb-hdrc musb-hdrc.1.auto: dma_pool_free buffer-128, f134e000/be842000 (bad dma)
hcd_buffer_create() creates a few buffers, the smallest has 32 bytes of
size. ARCH_KMALLOC_MINALIGN is set to 64 bytes. This combo results in
hcd_buffer_alloc() returning memory which is 32 bytes aligned and it
might by identified by buffer_offset() as another buffer. This means the
buffer which is on a 32 byte boundary will not get freed, instead it
tries to free another buffer with the error message.
This patch fixes the issue by creating the smallest DMA buffer with the
size of ARCH_KMALLOC_MINALIGN (or 32 in case ARCH_KMALLOC_MINALIGN is
smaller). This might be 32, 64 or even 128 bytes. The next three pools
will have the size 128, 512 and 2048.
In case the smallest pool is 128 bytes then we have only three pools
instead of four (and zero the first entry in the array).
The last pool size is always 2048 bytes which is the assumed PAGE_SIZE /
2 of 4096. I doubt it makes sense to continue using PAGE_SIZE / 2 where
we would end up with 8KiB buffer in case we have 16KiB pages.
Instead I think it makes sense to have a common size(s) and extend them
if there is need to.
There is a BUILD_BUG_ON() now in case someone has a minalign of more than
128 bytes.

Commit e9fd702a58c4 ("audit: convert audit watches to use fsnotify
instead of inotify") broke handling of renames in audit. Audit code
wants to update inode number of an inode corresponding to watched name
in a directory. When something gets renamed into a directory to a
watched name, inotify previously passed moved inode to audit code
however new fsnotify code passes directory inode where the change
happened. That confuses audit and it starts watching parent directory
instead of a file in a directory.
This can be observed for example by doing:

In audit log we see events like:

and that's it - we see event for the first touch after creating the
audit rule, we see events for rename but we don't see any event for the
last touch. However we start seeing events for unrelated stuff
happening in /tmp.
Fix the problem by passing moved inode as data in the FS_MOVED_FROM and
FS_MOVED_TO events instead of the directory where the change happens.
This doesn't introduce any new problems because noone besides
audit_watch.c cares about the passed value:
fs/notify/fanotify/fanotify.c cares only about FSNOTIFY_EVENT_PATH events.
fs/notify/dnotify/dnotify.c doesn't care about passed 'data' value at all.
fs/notify/inotify/inotify_fsnotify.c uses 'data' only for FSNOTIFY_EVENT_PATH.
kernel/audit_tree.c doesn't care about passed 'data' at all.
kernel/audit_watch.c expects moved inode as 'data'.

In commit be9f4a44e7d41 ("ipv4: tcp: remove per net tcp_sock")
I tried to address contention on a socket lock, but the solution
I chose was horrible :
commit 3a7c384ffd57e ("ipv4: tcp: unicast_sock should not land outside
of TCP stack") addressed a selinux regression.
commit 0980e56e506b ("ipv4: tcp: set unicast_sock uc_ttl to -1")
took care of another regression.
commit b5ec8eeac46 ("ipv4: fix ip_send_skb()") fixed another regression.
commit 811230cd85 ("tcp: ipv4: initialize unicast_sock sk_pacing_rate")
was another shot in the dark.
Really, just use a proper socket per cpu, and remove the skb_orphan()
call, to re-enable flow control.
This solves a serious problem with FQ packet scheduler when used in
hostile environments, as we do not want to allocate a flow structure
for every RST packet sent in response to a spoofed packet.

Not caching dst_entries which cause redirects could be exploited by hosts
on the same subnet, causing a severe DoS attack. This effect aggravated
since commit f88649721268999 ("ipv4: fix dst race in sk_dst_get()").
Lookups causing redirects will be allocated with DST_NOCACHE set which
will force dst_release to free them via RCU. Unfortunately waiting for
RCU grace period just takes too long, we can end up with >1M dst_entries
waiting to be released and the system will run OOM. rcuos threads cannot
catch up under high softirq load.
Attaching the flag to emit a redirect later on to the specific skb allows
us to cache those dst_entries thus reducing the pressure on allocation
and deallocation.
This issue was discovered by Marcelo Leitner.

When ak4114 work calls its callback and the callback invokes
ak4114_reinit(), it stalls due to flush_delayed_work(). For avoiding
this, control the reentrance by introducing a refcount. Also
flush_delayed_work() is replaced with cancel_delayed_work_sync().
The exactly same bug is present in ak4113.c and fixed as well.
